thrive.nel - Social Prescribing

What is Social Prescribing?

Social prescribing aims to connect people living in North East Lincs to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing.

thrive.nel has a team of Link Workers based here at Centre4, who are helping people to find the support they need, so if you’re considering a different way to improve your long-term health, Social Prescribing could be the right prescription for you!

How Does It Work?

We understand that it’s not always easy to do things differently. Working alongside your GP, an experienced Link Worker will spend time getting to know you on a confidential, one-to-one basis. Link Workers are practical and helpful and will not judge.

You can access support for up to 12 months and during this time your Link Worker will help you explore activities you may want to connect with and which could help you better manage your long-term condition and how you look after yourself.

What Support Is Available?

Some examples of the kind of activities your Link Worker can help you connect with include:

  • Finding out more and getting involved with social activities in your area
  • Rediscovering a hobby such as gardening and other creative interests such as arts and crafts
  • Befriending, counselling and access to other appropriate support to help you manage your condition better
  • Getting more active – access to walking groups, gym or keep fit classes
  • Advice about giving up smoking, losing weight and eating a bit more healthily
  • Advice about benefits, housing, debt and money management.

Eligibility

People in North East Lincolnshire area, aged between 18 & 75 and living with one or more of the Long Term Health Conditions shown below may be eligible for the thrive.nel program.

  • Asthma
  • Atrial Fibrillation
  • COPD
  • Chronic Heart Disease
  • Diabetes Type 1
  • Diabetes Type 2
  • Epilepsy
  • Fibromyalgia
  • Hypertension (high blood pressure)
  • Osteoarthritis
  • Osteoporosis
