What is Social Prescribing?

Social prescribing aims to connect people living in North East Lincs with community based activities that can help improve overall health, wellbeing and independence.

Our service is called thrive.nel and we now have a small team of Link Workers based here at Centre4.

If you are considering a different way to improve your long-term health, then Social Prescribing could be the right prescription for you!

How does it work?

We understand that it’s not always easy to do things differently.  Working alongside your GP, an experienced Link Worker will spend time getting to know you on a confidential, one-to-one basis.  Link Workers are practical and helpful and will not judge.  You can access support for up to two years and during this time your Link Worker will help you explore activities you may want to connect with and which could help you better manage your long-term condition and how you look after yourself.

What kind of activities and support are available to me?

Some examples of the kind of activities your Link Worker can help you connect with include:

  • Finding out more and getting involved with social activities in your area
  • Rediscovering a hobby such as gardening and other creative interests such as arts and crafts
  • Befriending, counselling and access to other appropriate support to help you manage your condition better
  • Getting more active - access to walking groups, gym or keep fit classes
  • Advice about giving up smoking, losing weight and eating a bit more healthily
  • Advice about benefits, housing, debt and money management.

How can I find out more?

If you are aged between 18 and 65 and living with one of the relevant long-term conditions* then we’d love to hear from you.

*Relevant long-term conditions are:

  • Asthma
  • COPD
  • Diabetes
  • Hypertension (high blood pressure)
  • Atrial Fibrillation
  • Chronic Heart Disease
  • Diabetes Type 1
  • Osteoporosis
  • Osteoarthritis
  • Fibromyalgia
  • Epilepsy
