Health & Wellbeing Collaborative Teams

Awareness Raising


This team focuses on raising awareness of the signs and symptoms of various cancers: lung, bowel, prostate, cervical and ovarian.  The aim is to encourage people to go to their doctor early to get checked out.  The volunteers also promote the National screening programmes.

Free Cancer Champions Training Course

Our Cancer Champions may have the opportunity to raise awareness and talk about the importance of early detection with their family, friends and social groups by providing information about symptoms of different cancers in an informal way.  The course if free of charge to attend with lunch provided and are usually held at Centre4.

Coronary Heart Disease

This team gets out and about in communities offering blood pressure tests opportunistically in community settings.  If it is discovered that you have high blood pressure, it can be managed by your GP with medication and lifestyle changes.  We are currently also offering risk assessments to people to see if they may be at risk of developing Type 2 Diabetes.

Skin Health

This team has worked closely with Specialist Tissue Viability Nurses in producing information about how to prevent leg ulcers and pressure sores.  Something which is little known about, but which can have devastating consequences.

Older People

The Older People’s Health & Well-Being and Good Neighbours team work in partnership with other agencies to assist older people in improving their Health & Wellbeing. 

Community Team members use a combination of a community-led social marketing and a collaborative approach to encouraging people to have a healthy lifestyle and reduce social isolation.

The team support 26 various clubs throughout North East Lincolnshire and support another 10 clubs from our partner agencies.

This team are a group of volunteers that take the difficult subject of Dementia awareness out into the community. The team talk to people in the community about Dementia and direct them to the most appropriate service.
