Building health and well-being together our Thrive team of link workers connect participants to community-based services to meet their unique needs and enhance overall quality of life.

What is Thrive?

Thrive is a social prescribing project that aims to improve your health and wellbeing by addressing practical, social, and emotional needs. Instead of relying solely on traditional medical treatments, it connects you to local activities, groups, and services that support your overall wellbeing.

This might include:

  • Community Activities and Support Groups
  • Mental Health and Well-being Support 
  • Practical Assistance Services
  • Lifestyle and Health Management Programmes

The goal of Thrive is to empower you to take control of your health by engaging with resources that promote a healthier and happier lifestyle. It’s facilitated by working with a link worker over a 12 month period who will help you identify your needs and connect you to appropriate community support.

Jodie looked at my finances and helped me apply for Attendance Allowance. This means I can now have my heating on whenever I want. Centre4 arranged all of this for me, and I’ll be forever grateful.

Are you Eligible?

Thrive is here for people aged 18 to 75 who are living with one or more of the following long-term health conditions:

  • Asthma
  • Atrial Fibrillation
  • Chronic Heart Disease
  • COPD (Chronic Obstructive Pulmonary Disease)
  • Diabetes Type 1 or Type 2
  • Epilepsy
  • Fibromyalgia
  • Hypertension (high blood pressure)
  • Osteoarthritis
  • Osteoporosis
