Centre4 is proud to be the home of the Social Prescribing initiative for North East Lincolnshire, offering a person-centred approach to health that supports the whole patient beyond clinical care. Our Social Prescribing team connects individuals with a range of non-medical resources in their community, addressing the social, emotional, and practical issues that influence overall well-being. 

Our Social Prescribing programmes offer access to community activities, support groups, mental health resources, and practical assistance services. We also provide lifestyle and health management programmes designed to support holistic wellbeing.

Participants benefit from reduced stress, improved mood, enhanced self-esteem, and increased physical fitness through gentle exercise. The programmes foster stronger community connections, helping to combat isolation and promote a sense of belonging.
